This Privacy Notice describes how LBee Health & LBee Health Practice Group, PLLC (“LBee Health,” “we,” “our,” or “us”) processes personal information (i.e., information reasonably related to a specific individual) we collect through our websites and other online interactions and communications with you (“Services”).
LBee Health is not a “covered entity” under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations, as amended from time to time (collectively, “HIPAA”). One or more of the healthcare providers or pharmacies we work with may or may not be a “covered entity” or “business associate” under HIPAA, and we may in some cases be a “business associate” of those healthcare providers or pharmacies. It is important to note that HIPAA does not necessarily apply to an entity or person simply because there is health information involved, and HIPAA may not apply to your transactions or communications with LBee Health or the healthcare providers or pharmacies you interact with through our platform. To the extent LBee Health is deemed a “business associate” however, and solely in its role as a business associate, LBee Health may be subject to certain provisions of HIPAA with respect to “protected health information” (as defined under HIPAA) that LBee Health handles on behalf of the covered entity healthcare providers or pharmacies (“PHI”).
You have certain rights under HIPAA if the healthcare provider or pharmacy that provides services to you through the Services is a “covered entity” under HIPAA. These rights include:

Obtaining a copy of your paper or electronic medical records from the healthcare provider or pharmacy

Asking your healthcare provider or pharmacy to correct your paper or electronic medical record

Requesting confidential communication from your healthcare provider or pharmacy

File a complaint if you believe your privacy rights have been violated by your healthcare provider or pharmacy
This Privacy Notice does not apply to any PHI or any other medical or health information that is not subject to HIPAA but is protected by state laws and that we collect as a service provider on behalf of healthcare providers or pharmacies that provide services to you through the Services. When we collect information as a business associate or service provider, we will only use and disclose the information in accordance with applicable laws and our agreements with those healthcare providers or pharmacies. Please contact your healthcare provider or pharmacy for more information about how this information is handled and, if applicable, to exercise your rights under HIPAA.
Changes to this Privacy Notice
We may update this Privacy Notice from time to time. Any updated Privacy Notice will be effective when posted. If required by law, we will contact you directly to provide you with an updated Privacy Notice. You are responsible for ensuring we have an up-to-date active and deliverable email address for you. The date the Privacy Notice was last revised is identified at the top of the page. Please check this Privacy Notice periodically for updates.

Pages of our Website may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit the Company, for example, to count users who have visited those pages and for other related website statistics (for example, recording the popularity of certain website content and verifying system and server integrity).
We use cookies and other tracking technologies to understand how visitors use our Services, to improve the functionality and features of our Services, and to measure and improve the effectiveness of our marketing and advertising. Web browsers often allow you to send a “Do Not Track” request with your browsing traffic to signal your request to not have data about your online browsing activities monitored and collected. We do not currently honor Do Not Track requests from web browsers.

However, no data transmission over the Internet can be guaranteed to be fully secure. We recommend that you take reasonable precautions to protect the personal information you send and receive from us.
7. Data Retention
We retain personal information as long as necessary to fulfill the purposes we have outlined above unless we are required to do otherwise by applicable law. Once you have terminated your relationship with us, we may retain your personal information in our systems and records in order to ensure adequate fulfillment of surviving provisions in terminated contracts or for other legitimate business purposes, such as to enable easier future user onboarding, demonstrate our business practices and contractual obligations, or provide you with information about our products and services in case of interest.
8. Children’s Privacy
Our Services are not intended for children under 18 years of age. The Services are not directed at, marketed to, or intended for children under 18 years of age. As a general rule, we do not knowingly collect personal information from children under 18. If you are under 18, do not use or provide any information on the Services or otherwise provide any information about yourself to us, including your name, address, telephone number, or email address. If we learn we have collected or received personal information from a child under 18 without verification of parental consent, we will delete that information. Our Services may contain links to external sites or other online services that we do not control, including those embedded in third party advertisements or sponsor information. We are not responsible for the privacy practices or data collection policies of such third-party services. 10. Nevada Residents
If you are a Nevada resident, you have the right to opt out of the sale of certain Personal Information, including your name and mailing address, to third parties.
11. California Residents

12. International Transfers of Personal Information
If you are located outside of the United States and provide personal information to us, your personal information will be transferred to the United States, where data protection laws may differ from those of the country in which you reside. By providing us with your personal information, you acknowledge that your personal information will be transferred to the United States and processed on servers in the United States. If you do not want to have you Personal Information transferred and processed in the United States, please do not submit it us.
